Habilitating Hands Inc. and Habilitating Hands Guardianship Services Inc. is a medical case management, consulting, and guardianship service. Our services are offered to the elderly and disabled individuals, as well as their family members. Our primary focus is to enhance and assist in providing the highest quality of life for our clients. These services are provided to Trust Officers, Probate Attorneys, Families, and the Probate Court system, by licensed nurses, clinicians and case managers/ care managers.
